Quest Location Screenings

How to create an appointment at one of Quest’s Locations:

• Log in to Personify Health
• Click Benefits
• Scroll down and click on Quest Diagnostics 
• Click START NOW 
• Under At a Patient Center
  • Click Schedule a Screening
• Enter your zip code and click Search
• Find the closest Quest Facility to you and click Select
• Find the best date & time for you, select it and then press Continue 
• Then review the information and click Confirm
